Improving patient care with in-situ simulation training

The simulation team at the hospital is now adding another dimension to staff training by running specifically tailored simulations away from the simulation suite and into the actual workplace, so that teams working together also train together, and so further help to improve patient outcomes.

Derriford Hospital has long been an advocate of simulation training and over the last five years has established a comprehensive timetable of simulation programmes for a wide variety of multi-disciplined hospital staff. With the latest addition of the wireless patient simulator - SimMan 3G, the simulation team at the hospital is now adding another dimension to staff training by running specifically tailored simulations away from the simulation suite and into the actual workplace, so that teams working together also train together, and so further help to improve patient outcomes.

Transferring learning to the workplace

Tom Gale, Consultant Anaesthetist gives an insight into some of the training at the hospital. “When training our theatre teams, for example, we ask them what critical incidents and near misses they have had in the last 6 months so that we can tailor the learning objectives to those particular scenarios for those particular people. Contextualising the simulation in this way makes the scenarios more authentic, and the lessons learnt are more easily transferred to the workplace. Ultimately, team performance can be strengthened and latent threats in the clinical environment identified to promote better patient outcomes if similar incidents arise again in the future.”

An exemplar of the recent Chief Medical Officer’s vision to embrace simulation as a means of ‘building a safer healthcare system’ (1), Derriford Hospital is continuing to expand its simulation programmes. Dr. Gale continues, “The mobile nature of SimMan 3G has enabled us to create simulated environments across the whole hospital. We can produce simulation learning in theatre environments, the Intensive Care Unit, the High Dependency Unit and the Emergency Department. We also have simulation programmes for neonatal training, for paediatrics, maternity and delivery suites. At Derriford Hospital, we believe that in-situ simulation is a perfect vehicle to bring simulation training to a wide variety of healthcare groups throughout the hospital.”

SimMan 3G Trauma



We tailored our SimMan 3G to create a trauma patient simulator specifically designed for military and civilian emergency services. The SimMan 3G Trauma is well suited for training the rapid assessment of trauma emergencies. It will also simulate necessary interventions such as hemorrhage control and airway management.

Quality CPR Feedback
Laerdal™s Q-CPR technology measures the quality of CPR providing real time feedback on compression rate, depth, release, and hands-off time as well as generating palpable pulses, blood pressure wave forms and ECG artefacts.

Convulsions
Degrees of seizures and convulsions can be created from minor effect through to a full convulsion through the Instructor Mode.

Bleeding and Wounds
Wound models can be connected to an internal blood reservoir which will bleed both from arterial and venous vessels. Connected to the simulator™s physiological modelling, SimMan 3G will react appropriately according to treatment.

Drug & Event Recognition
The new and advanced Drug Recognition System allows students to administer drugs simultaneously. It registers the amount, speed and type of drug automatically and applies the appropriate physiological respones, saving the instructor time and improving the overall intelligent debrief.

Eye Signs
Include pupillary responses to light, blinks at slow, normal and fast rates, winks and open, partially open and closed reactions.

Vascular Access
In addition to the standard vascular access in the right arm, the new intraosseous access via the tibia and sternum allows for procedure accuracy.

Realistic anatomical features:

 Realistic airway and breathing functionality :

  • The airway has been designed to allow for training in all aspects of newborn airway management, including the use of positive-pressure airway devices and the placement of ET tubes and LMAs.
  • Lung compliance: Instructors can alter the simulator’s lung compliance to challenge the provider’s skills in detecting and treating lung compliance changes.
  • Realistic chest rise: The chest rise on the simulator can be adapted to be absent or normal, with breathing rates up to 100 breaths per minute.
