A prospective, randomized, study comparing early percutaneous dilational tracheotomy to prolonged translaryngeal intubation (delayed tracheotomy) in critically ill medical patients.

Major Finding

Early tracheostomy improves mortality (NNT 3) and reduces incidence of pneumonia (NNT 5) when compared with late tracheostomy in critically ill medical patients.

Point for discussion:

On the basis of this evidence should early tracheostomy be encouraged in sick medical patients likely to need prolonged ventilation?
