Minneci et al. Meta-analysis. Ann Int Med 2004.

Created: 21/5/2007

Paper type

Systematic Review and meta-analysis

Minneci PC, Deans KJ, Banks SM et al.
Meta-analysis: The Effect of Steroids on Survival and Shock during Sepsis Depends on the Dose.
Ann Int Med. 2004; 141: 47-56

Major Finding

In patients with vasopressor dependant septic shock, 5 to 7 day courses of physiological dose hydrocortisone replacement increases both survival rates and shock reversal.

Point for discussion:

Although this meta-analysis is in agreement with that of Annane's et al  are we still certain that low dose steroids should be considered for severely ill patients with vasopressor-dependent septic shock? Has the recently completed CORTICUS trial finally answered the question?
