Improving patient care with in-situ simulation training

Derriford Hospital, United Kingdom
The simulation team at the hospital is now adding another dimension to staff training by running specifically tailored simulations away from the simulation suite and into the actual workplace, so that teams working together also train together, and so further help to improve patient outcomes.

Derriford Hospital has long been an advocate of simulation training and over the last five years has established a comprehensive timetable of simulation programmes for a wide variety of multi-disciplined hospital staff. With the latest addition of the wireless patient simulator - SimMan 3G, the simulation team at the hospital is now adding another dimension to staff training by running specifically tailored simulations away from the simulation suite and into the actual workplace, so that teams working together also train together, and so further help to improve patient outcomes.

Transferring learning to the workplace

Tom Gale, Consultant Anaesthetist gives an insight into some of the training at the hospital. “When training our theatre teams, for example, we ask them what critical incidents and near misses they have had in the last 6 months so that we can tailor the learning objectives to those particular scenarios for those particular people. Contextualising the simulation in this way makes the scenarios more authentic, and the lessons learnt are more easily transferred to the workplace. Ultimately, team performance can be strengthened and latent threats in the clinical environment identified to promote better patient outcomes if similar incidents arise again in the future.”

The value of In-Situ Simulation

An exemplar of the recent Chief Medical Officer’s vision to embrace simulation as a means of ‘building a safer healthcare system’ (1), Derriford Hospital is continuing to expand its simulation programmes. Dr. Gale continues, “The mobile nature of SimMan 3G has enabled us to create simulated environments across the whole hospital. We can produce simulation learning in theatre environments, the Intensive Care Unit, the High Dependency Unit and the Emergency Department. We also have simulation programmes for neonatal training, for paediatrics, maternity and delivery suites. At Derriford Hospital, we believe that in-situ simulation is a perfect vehicle to bring simulation training to a wide variety of healthcare groups throughout the hospital.”

SimMan® Essential/ SimMan Essential Bleeding

SimMan Essential offers comprehensive clinical functionally to teach the core skills of airway, breathing, cardiac and circulation management. SimMan Essential is easy to program, operate and transport. Gradient levels of difficulty can be applied to each scenario to build learner competence.

SimMan Essential Bleeding offers the same clinical features as SimMan Essential but also incorporates internal blood system offering unique benefits for trauma training. The Bleeding modules are developed to teach rapid identification and effective management of bleeding control.

Solutions for Military

SimMan 3G Trauma

We tailored our SimMan 3G to create a trauma patient simulator specifically designed for military and civilian emergency services. The SimMan 3G Trauma is well suited for training the rapid assessment of trauma emergencies. It will also simulate necessary interventions such as hemorrhage control and airway management.

From the SimMan 3G we added some essential features, such as amputated limbs and sternal IO access, but streamlined others so that the SimMan 3G trauma is focused on providing optimal training for trauma emergency situations.
These features, along with the rugged PC, wireless configuration and carry case, make SimMan 3G Trauma the quality choice for realistic training in any environment – whether in a hospital, an ambulance or in a military combat environment.

SimMan 3G®

Imagine the training benefits of running a complete and continuous medical scenario from the moment of patient injury, at any location, through the challenges of patient transportation and concluding in a hospital; all realistically facilitated by a single patient simulator.

Completely wireless and self-contained, simulation training need no longer be compromised by location and equipment constraints. Designed using the most cutting-edge technology, SimMan 3G opens up numerous simulation possibilities for variable locations and mobility. Easy to programme, operate and transport, all aspects of medical training can now be challenged simultaneously and gradient levels of difficulty can be applied to each scenario to build student competence.

SimMan 3G is compatible with Laerdal’s new Audio Visual system – ‘SimView’, which provides comprehensive debriefing support for quality feedback and effective learning.

Just some of SimMan 3G’s features include:

Quality CPR Feedback
Laerdal™s Q-CPR technology measures the quality of CPR providing real time feedback on compression rate, depth, release, and hands-off time as well as generating palpable pulses, blood pressure wave forms and ECG artefacts.

Degrees of seizures and convulsions can be created from minor effect through to a full convulsion through the Instructor Mode.

Bleeding and Wounds
Wound models can be connected to an internal blood reservoir which will bleed both from arterial and venous vessels. Connected to the simulator™s physiological modelling, SimMan 3G will react appropriately according to treatment.

Drug & Event Recognition
The new and advanced Drug Recognition System allows students to administer drugs simultaneously. It registers the amount, speed and type of drug automatically and applies the appropriate physiological respones, saving the instructor time and improving the overall intelligent debrief.

Eye Signs
Include pupillary responses to light, blinks at slow, normal and fast rates, winks and open, partially open and closed reactions.

Vascular Access
In addition to the standard vascular access in the right arm, the new intraosseous access via the tibia and sternum allows for procedure accuracy.

This is Laerdal’s first neonatal simulator and the youngest member of our patient simulator family. Its realistic traits and lifelike responses possibly make it the most emotionally engaging patient simulator of all.

Designed to meet the training requirements of neonatal emergency medicine and resuscitation courses, SimNewB is the most authentic and comprehensive simulation solution in its field.

Making simulation easier
Easy to integrate into all neonatal training curricula, SimNewB’s innovative user interface and scenario design allows instructors to control the simulator’s responses throughout a training session with the push of a button on the remote handheld control. Features include six preset patient levels ranging from a vigorous to a compromised newborn.

Helping you succeed
High-quality simulation training requires much more than a simulator. SimNewB is an all-encompassing simulation solution which includes course material, as well as our reputable Advanced Video System, which will assist you to give effective and comprehensive debriefing.

Instructor training courses are available to get you up and running quickly, while our range of technical services will look after the practical aspects of installing and maintaining your equipment.

Realistic anatomical features:
The simulator accurately represents a full-term, 50th percentile newborn female. Measuring 21 inches and weighing 7lbs, SimNewB can present as a limp, cyanotic newborn with no vital signs, or as a moving, crying, vigorous newborn.

Patient umbilicus

An umbilicus with a life-like pulse can be assessed, cut, and can be catheterized for IV access.

Intraosseous access

SimNewB features I/O access in both legs.

Realistic airway and breathing functionality :

  • The airway has been designed to allow for training in all aspects of newborn airway management, including the use of positive-pressure airway devices and the placement of ET tubes and LMAs.
  • Lung compliance: Instructors can alter the simulator’s lung compliance to challenge the provider’s skills in detecting and treating lung compliance changes.
  • Realistic chest rise: The chest rise on the simulator can be adapted to be absent or normal, with breathing rates up to 100 breaths per minute.
  • Needle decompression: The torso of SimNewB includes functionality to relieve a pneumothorax via needle decompression.
