A 74-year-old man presents with a lung malignancy. He is a smoker, and has a heart murmur, mild hypertension and diet-controlled diabetes mellitus.

You are provided with the results of the following investigations: spirometry, echo report, lateral C-spine X-ray.

He has experienced a sudden onset of neck pain, with no neurological signs.

1. Summarise the case.
2. Discuss how you would intubate a patient with a C2 dislocation.
3. Discuss diabetes and hypertension.
4. Discuss postdural puncture headache.
5. Describe how you would prepare for a thyroidectomy in this patient.
